What it means to use telehealth — the benefits, the limits, and what we are doing to keep you safe.
When you use ZYLRX to consult with a licensed medical provider, you're consenting to receive medical care via telehealth. This document explains what that means and what to expect.
Telehealth is the delivery of medical services using electronic communications — secure messaging, video, and data sharing — instead of in-person visits. Through the ZYLRX platform, you'll exchange information with a licensed medical provider electronically rather than face-to-face.
Telehealth is not appropriate for every situation. By consenting, you understand that:
Telehealth is not for emergencies. If you are experiencing chest pain, difficulty breathing, severe bleeding, suicidal thoughts, or any other emergency, call 911 or go to the nearest emergency room immediately. Do not attempt to reach your provider through ZYLRX for emergency care.
For telehealth to work safely, you agree to:
Your communications with your provider are confidential and protected by HIPAA. They are stored on encrypted, secure systems and shared only with the people directly involved in your care (your provider, the dispensing pharmacy, etc.). See our Privacy Policy for details.
Your telehealth interactions become part of your medical record. You have the right to access these records and request copies through your account dashboard or by contacting records@zylrx.com.
You have the right to refuse telehealth services or withdraw your consent at any time. If you do, your provider can help you find in-person alternatives. Withdrawing consent does not affect care you've already received.
Telehealth is regulated at the state level, and the rules vary. Your provider is licensed in your state and follows your state's specific telehealth requirements. If you move to a different state, please notify us so we can ensure your provider is properly licensed for your new location.
By submitting an intake or consultation request through ZYLRX, you acknowledge that:
If you have questions about telehealth or this consent before submitting an intake, contact us at support@zylrx.com or use our contact form.
This is generic placeholder content. Before launch, have a healthcare attorney review and customize this consent to comply with your specific state telehealth laws (which vary significantly), your provider network's requirements, and any specialty-specific consent requirements (e.g., for controlled substances).
